Rules are so those who join do not post idiotic things on the site.
Contact information is for those who want to contact you outside of the site
Affiliation banners are things you trade with other proboard sites to get members.
Codes make your site look cooler. HTML usually.

As for the rules, I would base them off of the Proboards terms of service. Since, they are God to us.
Such as;
PG-13 Content - Images, language, actions.
Also you have to let them know how they are allowed to treat each other, such as no flamming, or harassment of any kind.
Rules are probably the hardest thing, and they continue to grow as you think up new things that bother you.
